Health Care Connect Verification Process Underway

Ontario has a plan to connect all Ontarians who were on the Health Care Connect waitlist as of January 1, 2025 to a family doctor or nurse practitioner by Spring 2026.

You may receive an official letter, email or text message from Health Care Connect asking you to confirm if you are still in need of primary care and ensure we have up-to-date information. You can also reach out directly:
