Community Nursing Clinics

Qualified, registered nurses and health care professionals are available to provide you with a variety of nursing services, such as wound care, intravenous therapy or catheter care in a community nursing clinic.

  • Our clean and safe clinics follow strict Infection Prevention and Control protocols, providing optimal quality care. Clinics are stocked with medical supplies, resources, equipment, ideal lighting and examination tables.
  • There are 200+ locations across the province that provide quality nursing care.
  • Your care team will develop a customized care plan to meet your health care goals.
  • Learn to be independent. Our care plans support your independence. We will help you manage your own ongoing care, if deemed safe and appropriate.

Ontario Health atHome community nursing clinics offer a variety of services that vary by location and may not be available in all areas.

Please be punctual. Arriving on time for your appointment helps ensure that your visit runs smoothly and allows us to stay on schedule for all patients. Late arrivals may need to wait or be rescheduled, as we want to avoid delaying others who have arrived on time.

If you need to cancel or change your appointment, please provide as much notice as possible. Early notice allows us to offer the appointment time to another patient who may be waiting for care.

Help us provide the safest and most effective care. Please bring your current medication list, as well as any supplies or equipment you use at home to each appointment.

If you are receiving care at home, your care coordinator and nursing provider may work with you to transition your care to a nursing clinic.

Please contact your care coordinator if you have questions about Community Nursing Clinics.
