The Central East Centralized Diabetes Intake is a streamlined and integrated referral service for people living with or at risk of developing diabetes. Centralized Diabetes Intake (CDI) is provided by the Central East Local Health Integration Network (Central East LHIN) to link patients living with or at risk of developing diabetes to Diabetes Education Programs (DEP) or the Central East Centre for Complex Diabetes Care (CCDC). The Central East LHIN CDI care coordinator will determine the need for DEP or CCDC services. Together, the patient and care coordinator choose the most appropriate location for these diabetes services.
