Working closely with the Executive Leadership Team, Anna advances our vision of “exceptional care – wherever you call home” and strengthens Ontario Health atHome as a key partner in building an integrated health system for patients, families, and caregivers. Appointed CEO in 2025 after serving as Interim CEO, Anna has guided the organization with transparency, collaboration, and integrity, ensuring stability during transition while connecting directly with staff across the province. Chief Patient Services Officer
Lisa Burden leads the delivery of patient care services for Ontario Health atHome across the province. This portfolio is fundamental to our mission of assisting our patients to be healthier at home through connected, accessible, patient-centred care. Interim Chief Financial Officer (CFO)
Cindy Ward leads and provides direction and oversight for financial planning, financial controllership and management. With an incredible responsibility to the residents of Ontario, Cindy and her team manage approximately $4 billion of public funds. Cindy’s oversight defines and drives finance programs, policies, systems, services and models within the broader framework of Ontario Health atHome’s mandate and mission, vision and values. Lisa Tweedy provides strategic leadership, direction and oversight for the development, implementation and evaluation of human capital and organizational development strategies that promote system alignment and enable delivery of patient care. Tini Le
Chief Quality, Safety and Risk Officer
Tini Le leads several provincial teams, strategies and initiatives to drive patient safety and quality of care, exceptional organizational and health care specific policies and practices, and regulatory compliance to serve the needs of over 700,000 patients with highly diverse and complex needs across Ontario. She also leads the provincial long-term care placement program for people who are no longer able to reside at home, contributes to system modernization planning, and serves as the Regional Lead for patient services teams across Central Region.
