Health Care Connect Verification Process Underway

Ontario has a plan to connect all Ontarians who were on the Health Care Connect waitlist as of January 1, 2025 to a family doctor or nurse practitioner by Spring 2026.

You may receive an official letter, email or text message from Health Care Connect asking you to confirm if you are still in need of primary care and ensure we have up-to-date information.

Please advise if you would like to remain on the waitlist for a family doctor or nurse practitioner or if you would like to be removed from the waitlist and include the following information in your message:

1.     Your First and Last Name

2.     Phone Number

3.     Home Address

4.     Email address

5.     Reason for your call or email

  • For example, you would like to be removed from the waitlist because you have found your own family doctor or you have moved and no longer require a family doctor.
  • You would like to remain on the Health Care Connect waitlist for a family doctor or nurse practitioner.
  • Your contact information has changed.

If you confirm or update your information online at Ontario.ca/HealthCareConnect or if you have previously confirmed your information, please send us a quick email or voice mail message to inform us that your information has been updated. We will remove your name from our verification list to avoid any additional calls or emails in the future.

If you have already contacted Health Care Connect to confirm your information, no other action is required at this time. You may disregard additional communication from Health Care Connect related to updating your information.
