Acclaim Health Privacy Incident

Ontario Health atHome has been advised by Acclaim Health, one of our contracted home care service providers, of an incident involving unauthorized access to patient information in Mississauga Halton.
Acclaim Health patients in this area will receive notification about the incident directly from Acclaim Health while the investigation continues.
Protecting the privacy and security of patient information is a priority, and we are working with Acclaim Health and relevant partners, including Ontario’s Information and Privacy Commissioner.
