Quality Framework The provincial Ontario Health atHome Quality Framework was developed to ensure a c onsistent and coordinated approach to delivering quality services and serves as a foundation to hel p staf and service provider organizations improve caregiver and patient care experience by providing s afe, efective, reliable care, improving the health of populations by focusing on prevention and wellness, dec reasing cost, and improving the provider/staf and patient/caregiver experience of care provisi on. The Quadruple Aim The Quality Framework establishes the patient and their caregivers as our prim ary focus, surrounded by a Quadruple Aim which defnes quality for Ontario Health atHome and sets direction . The Aims serve as the foundation for monitoring quality, guide the areas of focus, the priorities, the measu res of progress and reporting, and facilitate communication both internally and externally. T he aims are: 1. Enhancing Patient Experience as defned by the Institute of Medicine (2001) and Health Quality Ontario in all six dimensions of quality: safe, efective, patient- center ed, efcient, timely, and equitable health care for all citizens. 2.
