Le présent document s’adresse aux patients qui envisagent un séjour pour soins de relève de courte durée dans un foyer de soins de longue durée ainsi qu’à leurs aidants. Les renseignements ci-après vous aideront à prendre une décision, car ils indiquent ce qu’offrent aux résidents les différents programmes de soins de relève de courte durée.
Nous encourageons fortement les patients et les aidants à effectuer des recherches sur les foyers qui les intéressent et à les visiter. Il est important d’obtenir tous les renseignements pour aider vous et votre famille à prendre des décisions éclairées concernant les foyers qui peuvent répondre aux besoins de votre être cher.
Le Programme de soins de relève de courte durée offre un répit aux aidants en leur donnant l’assurance que leur être cher reçoit les soins nécessaires, sur une base temporaire, dans l’un des foyers nommés ci-après. Santé à domicile Ontario gère et confirme les séjours en fonction de la disponibilité des lits.
